What is the criteria for certification?

Proof of business residency in area requesting certification... 

Completed U.S. PLY, INC. StarCertified™, or StarChoice™ application...

• Currently insured (General Liability Insurance and Worker's Comp ), and must provide copy to be kept on file at all times...
• Proof of financial responsibility (past 2 years financial records and/or certified banking institution letter)...
  
• Members of, or affiliated in at least three (3) other industry related trade organizations...
• Minimum 3 years in business...
• Minimum 3 trade references (Distributors, etc...)
  
• Minimum 3 projects in area of requested certification (must supply photos of each)...
  
• Willingness to attend specialized training sessions onsite or web-based, at Manufacturer's discretion...
• Must promote only U.S. PLY, INC. related products on U.S. PLY, INC. provided leads...
• Agree to full non-disclosure agreement as it pertains to U.S.PLY, INC. products and services...
  
• No outstanding issues with local (areas worked) BBB...
• U.S. PLY, INC. local sales representative support (signed and dated enrollment application)

What would cause us to remove a StarCertified™, or StarChoice™ Applicator from the program?

• Misrepresenting U.S. PLY, INC., or the U.S. PLY, INC.  product line, warranties, etc...  and installing other (competing) systems in place of U.S. PLY, INC.
• Poor customer service...  (not responding to customer needs, requests, or service calls)...
  
• Not following up on sales leads provided by U.S. PLY, INC. or any affiliated U.S. PLY, INC., Distributor...
• Delinquent in payments to U.S. PLY, INC. and/or any affiliated U.S. PLY, INC., Distributor...
• Not following the guidelines of 100% commitment to U.S. PLY, INC.
• Incomplete, or unwillingness to complete site inspection punchlists (in a timely manner)...
